I work with a ton of active adults, and a handful are tennis, pickleball, and other racquet sport players.

And I can’t tell you how many times I hear this:

“Gulp... please don’t tell me you think it’s... a... rotator cuff tear, do you?”

It’s like the dreaded death sentence of shoulder pain.

News flash! Not all rotator cuff pain is a tear. Sometimes it’s just the muscles complaining because they’re being overused (that’s what we call tendinitis or tendinopathy).

Even if an MRI does show a tear, that doesn’t automatically mean surgery. Many times, the rotator cuff can still respond beautifully to strengthening and targeted treatment. And sometimes, that tear on the MRI isn’t even what’s causing the pain, it’s just... there.

In fact, a 2019 study (Lawrence et al.) found that up to 50% of people with rotator cuff tears had zero symptoms. Yep! Half the participants had a tear and didn’t even know it.
